HOW TO FILE YOUR COMPLAINT WITH BUSINESS EXPRESS

BEFORE YOU BEGIN:
Be sure it was not just a misunderstanding, or an inexperienced employee that mishandled your purchase. Be sure you have made a sincere effort to settle your complaint directly with the company by contacting the owner, manager or someone else in authority at the company.

Business Express is a privately owned, for-profit organization, not a government agency. We will attempt to help you and the company to reach a satisfactory settlement, even to the point of adjustment, if deemed necessary through arbitration.

Business Express does not handle complaints already being handled by an attorney, or other agency. If another agency is already working on this, do not complete form, but advise Business Express for its records.

INSTRUCTIONS:
1. Download and complete the complaint form in legible, businesslike manner. The company will receive a copy of this complaint. Therefore, please make sure your statement is complete and factual, but as brief as possible.
2. If more room is needed, use additional plain paper.
3. Answer all questions, which apply to your problem, as best you can.
4. Sign and date the form.
5. Return the form to Business Express.
6. Keep a copy for your records.
